You need another form too. Bruce recently emailed me this one with the
following instructions:

To renew through the club please fill out the attached "ARRL membership"
> form with your information.
> Then fill in the date on the attached "Club Commission Reconciliation"

> Mail the two forms to the address at the bottom of the Club Commission
